What are custom jaw implants?

Custom jaw implants are designed around your specific facial skeleton, not chosen from a size catalog. Dr. Zelken uses PEEK (polyether ether ketone), a biocompatible material with decades of clinical use in craniofacial and orthopedic surgery.

The Endomandible™ is Dr. Zelken's proprietary single-incision technique: wraparound jaw enhancement that improves jaw angle definition, lower-face width, and profile projection in one surgery.

What makes this different is that Dr. Zelken personally designs your implant using your own imaging data, then manufactures it in his on-site Newport Beach laboratory. No third-party implant company is involved.

These are patient-specific devices fabricated under Dr. Zelken's direct clinical care. They are not commercially distributed and are not currently FDA-approved for this specific application.

What sets this apart

1

Surgeon-designed, not engineer-designed

Dr. Zelken personally designs each implant. Most custom implant workflows involve a third-party engineering firm. Not here.

2

On-site CBCT and manufacturing

Imaging, design, and fabrication all under one roof. Implants can be ready in under a week once approved.

3

PEEK, not silicone

Biocompatible, radiolucent, and long-term stable. Used in cranial and orthopedic reconstruction for decades.

4

AAAASF-certified surgical facility

Surgery is performed in a nationally accredited surgical facility, not a procedure room.

What to expect

Jaw implant recovery: a realistic timeline

Most patients are socially presentable within 2 to 3 weeks. Final results continue to refine over several months.

Days 1 to 3
Immediate
Swelling, tightness, and numbness are expected and normal. Rest and limited activity. Pain is manageable with prescribed medication.
Week 1
Early healing
Rest and light walking. Sutures removed if external. Follow-up with Dr. Zelken, in-person or virtual.
Weeks 2 to 3
Early return
Swelling begins to improve. Most patients are comfortable in social and professional settings by the end of week 3.
Weeks 4 to 6
Active return
Most patients return to full activity including exercise and public speaking. Structural results become increasingly visible.
Months 2 to 6
Refinement
Swelling fully resolves. Soft tissues adapt around the implant. Temporary numbness typically resolves by month 3.
Long-term
Final result
PEEK implants are stable and long-lasting. Structural improvements are designed to be permanent.

No two recoveries are identical. Each patient's timeline varies based on anatomy and implant complexity. Dr. Zelken provides personalized guidance throughout.

Common questions

Frequently asked questions about jaw implants

Will I look unnatural after jaw implant surgery?
The look of an implant has far more to do with design than with the procedure itself. Because Dr. Zelken designs your implant personally around your anatomy and goals, the result is proportional to your face. The goal is structural improvement that looks like you, not a different person.
What is a PEEK jaw implant and is it safe?
PEEK is a high-performance polymer with an extensive track record in craniofacial and orthopedic surgery. It is biocompatible, radiolucent, and chemically stable. While not currently FDA-approved for this specific aesthetic application, it has been approved for other surgical uses for decades and is well-established in the surgical literature.
How is this different from a stock jaw implant?
Stock implants come in standard sizes trimmed to approximate your anatomy. Patient-specific implants are designed from your actual CT scan to fit your skeleton precisely. Dr. Zelken also designs the implant himself, not a third-party manufacturer, so the design logic is surgical and aesthetic rather than commercial.
How long does the entire process take?
Typically 4 to 6 months from initial consultation to surgery. Because manufacturing is done in-house, it is possible to design and fabricate your implant in under a week once your imaging is confirmed and the design is approved. The timeline depends on your scheduling, not manufacturing delays.
I have had prior jaw or chin surgery. Am I still a candidate?
Yes. Revision cases including prior genioplasty, orthognathic surgery, V-line reduction, or a prior implant that missed the mark are a significant part of Dr. Zelken's practice. Patient-specific planning is particularly well-suited for complex anatomy where standard implants will not work.
Why does it cost more than a standard implant?
The cost reflects the surgeon's time designing your implant, the medical-grade PEEK material, and in-house manufacturing equipment. Compared to custom implants sourced from third-party manufacturers, Dr. Zelken's approach is often comparable in cost while delivering a shorter lead time and a design that comes directly from your surgeon.
Is a virtual consultation available?
Yes. Initial consultations can be conducted virtually. A CBCT scan can be obtained at a facility near you and reviewed remotely by Dr. Zelken. An in-person examination is required before surgery. Virtual consultations are a practical option for patients traveling from outside Orange County.
What risks should I know about?
Risks include swelling, bruising, temporary numbness or weakness, and the potential for infection. Specific concerns with jaw implants include masseter muscle adaptation and the small possibility of implant-related complications. Dr. Zelken discusses all risks in detail during your consultation and follows every patient closely through recovery.
